A2M is a circulating 720 kDa homotetramer expressed in the liver which captures a wide range of plasma proteinases including plasmin and thrombin. Each monomer contains an internal thiol ester, a transglutaminase reactive site, zinc and receptor binding sites, and a bait region which when cleaved induces a conformational change trapping the proteinase. Serum A2M levels are decreased in acute pancreatitis and increased in chronic liver disease and nephrotic syndrome.
Is able to inhibit all four classes of proteinases by a unique 'trapping' mechanism. This protein has a peptide stretch, called the 'bait region' which contains specific cleavage sites for different proteinases. When a proteinase cleaves the bait region, a conformational change is induced in the protein which traps the proteinase. The entrapped enzyme remains active against low molecular weight substrates (activity against high molecular weight substrates is greatly reduced).
